Photodynamic Therapy and Cryotherapy for Oral
CHIANG, CHUN-PIN
ABSTRACT
Oral leukoplakia, erythroleukoplakia and papilloma & verrucous hyperplasia are the top 3 oral precanceroses in Taiwan. We began to use photodynamic therapy of partially applying ALA (5-aminolevulinic acid) (ALA-PDT) of 635 nm LED red light to treat 36 cases of oral papilloma & verrucous hyperplasia approximately 12 years ago. The pathological change completely disappeared after the treatment. Among the 20 cases of oral erythroleukoplakia which had been treated, 17 had complete response, and 3 had partial response. Among the 65 cases of oral leukoplakia treated, 5 had complete response, 33 had partial response, 27 had no response. We used partial ALA-PDT of 635mm laser red light again to treat 40 cases of oral papilloma & verrucous hyperplasia. Following the treatment, we found that the pathological change was completely gone. Among the 40 cases which had undergone an oral erythroleukoplakia treatment, 38 had complete response, and 2 had partial response. Among the 40 cases of oral leukoplakia treated, 7 had complete response, 20 had partial response, and 13 had no response. From the above results, it was noticeable that partial ALA-PDT treatment was very effective over oral papilloma & verrucous hyperplasia and oral erythroleukoplakia, but less so on oral leukoplakia. Hence, we performed a swab liquid nitrogen cryotherapy on 60 cases of oral leukoplakia. In the wake of 6.3 times treatment per case on average, all showed complete response. Besides, we performed a spray gun liquid nitrogen cryotherapy on another 60 cases of oral leukoplakia. Following 3.1 times treatment per case on average, all demonstrated complete response. Therefore, we found that in respect of oral precancerosis treatment, in addition to surgery and laser resection, partial ALA-PDT therapy and swab and spray gun liquid nitrogen cryotherapy are two important options.
