    Background: Oral hypofunction has been identified as a critical factor that increases the risk of requiring long-term care and overall mortality. Recognizing this significance, the Japanese Ministry of Health, Labor and Welfare designated it as an insurance-covered disease in 2018. Current therapeutic approaches primarily focus on myofunctional training, such as “Tongue Rotation” and “A-I-U- Be” exercises, to enhance the musculature governing oral functions.

    Pioneering Practice in Shiki City: Shiki City in Saitama Prefecture has been a national pioneer in implementing oral function management programs. The speaker has been deeply involved in these initiatives as a leading expert, establishing a model for community-based oral frailty prevention.

    The Missing Link – TMJ Function: While muscle training is essential, clinical evidence suggests that aging-related physiological changes in the temporomandibular joint (TMJ) significantly reduce the range of motion, thereby impairing masticatory efficiency. Despite this, TMJ management remains largely overlooked in the current protocols for oral hypofunction.Objective and Discussion: This presentation discusses the scientific rationale for integrating TMJ functional recovery into existing oral function management. Drawing from the “Shiki Model” and clinical research, the speaker will argue that restoring TMJ mobility is a fundamental prerequisite for maximizing the effectiveness of conventional muscle training. Furthermore, the discussion will address the necessity of including “TMJ Management” in public health insurance policies in both Japan and Taiwan to address the challenges of an aging society.

    As dental implant treatment has become more widespread, it is now regarded as a common treatment method for addressing tooth loss. Since dental implants are placed into the extracted alveolar bone, the condition of the alveolar bone and the amount of available bone determine the difficulty of the procedure. Securing sufficient hard tissue around the implant mainly influences its long-term stability. The condition of the residual alveolar ridge varies among patients depending on the cause of extraction and the time elapsed before implant placement. Placing implants in severely resorbed alveolar bone and creating new bone is generally very challenging, requires a long treatment period, and demands careful selection of graft materials and barrier membranes. In this lecture, we will discuss considerations for hard tissue reconstruction in patients with moderate to severe horizontal and vertical bone defects, surgical precautions, and methods to simplify complex and difficult procedures.

    In Japan, oral hypofunction in older adults and oral function developmental disorder in children were officially established as diagnostic categories covered by the public health insurance system in 2018. In addition, the Ministry of Health, Labour and Welfare has clearly emphasized that the role of the family dentist extends beyond the management of dental caries and periodontal disease to include the acquisition, maintenance, and restoration of oral function. As a result, the importance of oral function management has gained increasing attention.

    Assessment of oral hypofunction is conducted using seven examination items: poor oral hygiene, oral dryness, decreased occlusal force, reduced tongue–lip motor function, low tongue pressure, impaired masticatory function, and impaired swallowing function. Individuals who meet three or more of these criteria are diagnosed with oral hypofunction and are managed through comprehensive oral function management. However, unlike diseases such as dental caries and periodontal disease, which are associated with visible structural changes, oral function represents an intrinsic and functional condition. Consequently, many dental professionals find it challenging to form a concrete clinical image of oral function management.

    In this lecture, methods for assessing oral function in older adults, interpretation of assessment results, and practical approaches to oral function management will be explained. Actual clinical cases will also be presented to illustrate real-world application.

    摘要

    「眼所能見、手才能及」幾乎是許多根管治療醫師的心聲,牙科操作型顯微鏡的出現更讓根管治療從傳統的瞎子摸象憑感覺式的治療,迅速拔高到一目了然的精緻化工程!所以當你在顯微鏡下搭配各式超音波器械做治療時,自然就不容易犧牲過多齒質,「微創根管治療」的目標也就能輕易達成。
    此外,近三十年對牙醫師作根管治療影響最大的就是鎳鈦旋轉器械的出現,它讓牙醫師們既期待又怕受傷害:期待的是藉由它的高彈性及高效率應付彎曲根管的修形,但又不會無預警地在根管中斷裂。隨著原廠專利保護的到期,幾乎主要研發各國都紛紛推出經由特殊熱處理的新式鎳鈦金屬合金,其最大目標就是要提升器械的抗疲勞斷裂強度,同時簡化操作步驟,增加安全性及切割效率,並搭配專屬的馬來膠針及根管封閉劑,讓根管治療一氣呵成,大幅降低術後的疼痛感,提高病人滿意度。
    所以本次演講的目的,就是希望藉由李偉明醫師專業又幽默風趣的表達,經由許多案例幫大家做深入淺出的解析,上完這堂課,將學會牙科顯微鏡的操作小技巧、超音波器械的挑選與使用時機、聰明運用鎳鈦旋轉器械、 新式電動馬達的操作與設定模式,以及根管治療後的牙齒修復策略,進而建立堅實的立論基礎,讓根管治療能進行得更順暢,實現真正的「精準醫療」,不僅大幅提升根管成功率,更讓牙齒不再被輕易拔除。

    現代牙科治療中,單純追求牙齒排列整齊已不足以滿足患者對「完美笑容」與「全口功能重建」的需求。本演講以「From Alignment to Perfection」為主題,探討如何從正確的齒列矯正出發,整合修復、咬合重建與美學設計,達到全面性與長期穩定的治療結果。

    This presentation by Dr. Tarin Piangsuk, American Board Certified Prosthodontist, will explore the mechanical properties, bonding mechanisms, and clinical applications of Midas, with a possibility to finish onlays, veneers, and quadrant restorative dentistry within one appointment.

    3D printed resin ceramic has been formulated to combine the optical and mechanical advantages of glass ceramics with the adaptability of resin-based materials. Its densely packed ceramic particles within a polymer matrix enhance flexural strength, fracture toughness, and wear resistance, making it suitable for both posterior functional and anterior esthetic applications.

    Key mechanical parameters—including biaxial flexural strength, elastic modulus, and fracture load—will be discussed in comparison with lithium disilicate and resin nano-ceramics. Attention will be given to the role of the ceramic filler network and polymer interface in resisting crack propagation and fatigue failure under masticatory loading. These characteristics allow Midas ceramic crown resin to maintain structural integrity even at reduced thickness, supporting minimally invasive restorative designs.

    A critical distinction in the clinical performance of restoration lies in the difference between bonded and non-bonded restorations. Bonded restorations demonstrate significantly higher fracture resistance and load-bearing capacity due to the adhesive interface that unites the restoration and the underlying tooth structure into a single biomechanical unit.

    In contrast, non-bonded restorations rely primarily on material strength and geometric retention, resulting in higher stress concentration and an increased risk of marginal or cohesive failure under cyclic load. The presentation will review data illustrating how proper adhesive bonding increases the overall flexural strength and distributes occlusal forces more evenly across the tooth-restoration complex. Understanding this distinction is crucial for clinical decision-making—particularly when selecting preparation design, material thickness, and bonding protocol for 3D printed resin ceramic restorations in posterior quadrants.

    Key workflow considerations—such as preparation geometry, print orientation, support placement, and curing protocol—will be outlined. Emphasis will be placed on how quadrant-based digital planning reduces occlusal discrepancies and enables harmonious occlusion across multiple restorations fabricated simultaneously.

    Sprintray Midas reflects a shift toward digitally driven, adhesive-based restorative dentistry. Continuous innovations in filler technology and resin chemistry are expected to further improve its mechanical and adhesive performance. By combining strength, esthetics, and speed, Midas empowers clinicians to perform chairside in a single visit, bonded quadrant restorations that rival laboratory-fabricated ceramics in both quality and reliability.
